CFMC, Inc. is a Federally Qualified Health Center with community health center locations throughout south central and a portion of north central Kentucky. From two small clinics in 2007, Cumberland Family Medical Center, Inc. has quickly grown into 158 locations across 30 counties with over 200 providers. Driven by health care needs of the population and fueled by multiple health disparities, we have evolved into one of the most progressive and fast growing community health centers in Kentucky.

Each year, through more than 321,000 patient visits, our health centers provide a full range of coordinated care to patients and families.

Excellence in Health Care

The community health centers of Cumberland Family Medical Center, Inc. (CFMC) were recognized by the Health Resources Services Administration (HRSA) in August 2017 as a “National Quality Leader.” CFMC was the only National Quality Leader designee in Kentucky and one of only 47 nation-wide to receive this distinction for their quality of patient care. This recognition places the clinics of CFMC in the top 1-2% of all Federally Qualified Health Centers (FQHC) in the nation.

To qualify for this distinction, community health centers must exceed national clinical benchmarks (Healthy People 2020 objectives and health center national averages) for chronic disease management, preventative care, and perinatal/prenatal care.

Currently there are 23 community health centers in Kentucky with 232 locations; nationwide, there are approximately 1375 community health centers with 9700 clinic locations.

Several CFMC, Inc. clinics have been awarded recognition as a Level 3 Patient Centered Medical Home (PCMH) by the National Committee on Quality Assurance (NCQA). As well, nine of our providers have been recognized as Level 3 PCMH providers by the NCQA.

Cumberland Family Medical Center, Inc. is dedicated to providing complete primary care services for the entire family, we also provide oral health services, school-based health centers in our local schools, a pediatric wellness program, a workplace wellness program, behavioral health services, local scholarship opportunities, outreach services, a 340B discount pharmacy program, and both pediatric and OB/GYN specialty care.
